South Central Jurisdiction (SCJ)
The following forms are needed for payment or reimbursement of expenses related to Jurisdictional business:

SCJ – United Methodist Volunteers in Mission (UMVIM)
There are several forms used by Volunteers in Mission team leaders and team members.

Forms required to be submitted by UMVIM Team Leaders
(Note: all of these forms are provided to leaders as he/she completes UMVIM Team Leader Training, scheduled through his/her respective Annual Conference Coordinator)

  • UMVIM Team Leader Application Form
  • UMVIM Team Leader Reference Form
  • Building a Budget Guideline
  • UMVIM Team Leader Checklist
  • UMVIM Team Leader Evaluation Form and Project Report
  • Host Evaluation Form
  • Local Pastor’s Evaluation of UMVIM Team (optional)
  • Record of Donated Assets (optional)
  • Suggested Format for Press Release(s)
  • Mission Cancellation Insurance Declaration Form (optional)

(Note: if the UMVIM Team’s project is a medical mission, then two additional forms must be completed)

(Note: all of these forms are provided to individual team members by their UMVIM Team Leader)

  • Agreement with Mission Policy Form
  • Team Member Application Form
  • Team Member Medical and Liability Release Form
  • Team Member Medical Information Form/Physician’s Statement
  • Team Member Packing List
  • Team Member Emergency Contact Information Form
  • In the Event of Death Form

(Note: if the individual team members are minors (under 18), then the following two forms–also supplied by the Team Leader–are required)

  • Parental Consent Form
  • Power of Attorney Form

Forms required to be submitted by Disaster Response Team Leaders
(Note: Provided to leaders as he/she completes Disaster Response Training, scheduled through his/her respective Annual Conference Coordinator)

  • Copy of Disaster Response Manual, which includes all required forms
